The Healthcare Coordinator is responsible for program integration and synchronization of care between Sharecare program participants and client-specific vendor partners.
*This is a remote position that can be located anywhere within the United States.
Coordinates the care and utilization of resources for large employer group and health plan participants in an appropriate, efficient and cost-effective manner while ensuring high quality care and the fulfilment of client-specific service level standards and performance guarantees* Evaluates and supports health coaching program participants in conjunction with the vendor partner personnel as necessary* Provides support to Sharecare's Clinical Operations Liaison, and Account Management teams including producing client-facing reporting recommending beneficial design and implementation changes for clients, and facilitating periodic training to health coach and clinical coach teams* Maintains inbound and outbound referrals, vendor partner communication, and prepares monthly, quarterly and annual inbound/outbound referral reporting
Oversees the Tobacco Surcharge process for contracts that require vendor/health plan coordination and reporting support * Maintains the Vendor Portal as an administrator; ensuring appropriate access by frontline staff and vendor partners and the protection of PHI
Grand rounds (Complex Case) facilitation and administrative work. Can include publishing agendas, compiling grand rounds (Complex Case) nominations summaries, preparing discussion notes, facilitating grand rounds discussions, coordinating intra vendor partner phone calls to support the grand round process in addition to the monthly all vendor partner operations call* Responsible for delivering follow-up communications to/from Sharecare's clinical/health coaches to/from client specific vendor partners
Provide updates to clinicians and coaches on related client participants with information received via the CIS* Collates reported vendor relations or process issues raised to the account managers for review on the weekly, biweekly and monthly operations call* Responsible for maintaining resources and communicating up-to-date information regarding vendor partners and the referral process via monthly newsletters and a dedicated operations Microsoft Teams site* Participates in quality improvement activities including, but not limited to, monthly call monitoring and documentation audits to ensure adherence to the tobacco surcharge and vendor referral process* Provides leadership with monthly reporting of frontline staff referral practices and improvement opportunities
Identifies and removes barriers that are interfering with the participant receiving appropriate care and integrated services
Follows-up on client specific participant complaints related to vendor referrals and the tobacco surcharge process
